Ford Military Recognition Program Proof of Eligibility
To ensure the exclusive rewards are directed rightfully to the deserving personnel, we request proof of military service. Rest assured that the verification process is quick and simple, so you can get straight to selecting your new vehicle. Verify military eligibility using one of these items listed below:
- Active Duty Service Members: Leave & Earnings Statement (LES), Active duty Enlisted Record Brief (ERB)/Officer Record Brief (ORB), Enlistment Contract
- Veterans: DD214 (Discharge Details Form 214), NGB form 22 (National Guard Bureau similar to a DD214), Veterans Health Identification Card (VHIC), or Veteran Identification Card (VIC), Driver's License or DMV-Issued State ID with VETERAN endorsement
- Retirees: Retiree Account Statement (eRAS), DD214 or NGB form 22 (must show 20 years of service or the reason for discharge was retirement)
- Surviving Spouses & Household members: upload any of the military documents listed in the categories above. Surviving Spouses can also submit a Casualty Report (DD Form 1300).
